e-Decorating Forms
You can find all the forms for your e-Decorating project here. For your convenience, you can submit all forms and photos online. You may also purchase your design online through PayPal using the "Buy Now" option on the e-Decorating page. If you're paying by check, you'll receive the mailing address once you submit the Client Information Sheet.
If you live in the Phoenix area you'll only need to submit the Client Information Sheet. Measurements and photos will be taken during the consultation.
